Step 2
Intake interview
When you come to us for an appointment, you will have an intake interview. If you want, you can bring someone to accompany you. The doctor will make an ultrasound to determine exactly how long you have been pregnant. You will also discuss with the doctor or nurse what your situation is, what treatment method is possible and any doubts you may have. You will also discuss which contraception you want to use after the treatment.
If you still have doubts about whether or not you want to terminate the pregnancy, we can refer you to the FIOM. The FIOM can support you in making your decision.
If you do decide to terminate your pregnancy, you can choose from 2 treatment methods: curettage or the abortion pill. If you are 9 weeks or more pregnant, then only curettage is possible.
Usually it is possible to carry out the treatment you choose to undergo following the intake interview. If this is not possible, a new appointment will be made with you.
What do you bring?
To your appointment, bring at least the following:
- A valid form of identification (passport, driver’s license or ID card)
And if necessary or possible:
- if you have: a card with your blood type
- Your health insurance card (if your BSN number is not on your ID)
If you want to have an IUD inserted or an implanon rod placed after curettage, bring it with you or buy one from us. You do not have to pay for the insertion of the IUD or rod if you have a BSN number. Please contact us for current information on fees.
In some cases, your health insurance will reimburse the cost of an IUD. Contact your health insurance for more information about possible reimbursement and any conditions they may have. Step 1
Choosing local anesthesia or sedation
Treatment with curettage is possible from the moment you are 9 days past your due date. It is an intervention in our clinic in which your uterus is emptied. To minimize the pain, you can opt for local anesthesia or sedation. Sedation means that you are briefly put to sleep. You have the choice between anesthesia or sedation only if you are no more than 13 weeks pregnant. After that, you always get sedation.
Local anesthesia
A local anesthetic means that you get a small prick at your cervix. As a result, you won’t feel any pain in that area. However, the inside of your uterus is not numb, so the treatment is not completely painless. You may experience abdominal cramps, like during your period. Also, some women find the sound of the curettage unpleasant.
If you choose a local anesthetic, you can eat and drink as usual beforehand. The preparation for the treatment can start immediately after the intake interview
Sedation
Sedation is performed by an anesthesiologist. You will then be administered a drug through an IV, which will put you briefly into a deep sleep. You will not feel or hear anything about the curettage. After the curettage you will wake up quickly.
If you choose sedation, you may not eat or smoke from 6 hours before the treatment. You may only drink 1 glass of water. From 2 hours before the treatment, this is also no longer allowed. Preliminary physician examination (only if sedation is chosen)
If you have chosen sedation, you will first have a preliminary examination with the doctor. He or she will ask you a number of questions about your health and your use of medication. He or she does this to check whether the drug used for the sedation is compatible with any medications or conditions you may have. If the doctor determines that sedation is possible for you, the curettage can take place immediately or a few days later. Step 3
The curettage
The curettage (also called “suction curettage”) takes place in our clinic. You will stay here for a total of about 3 hours. This time may be longer if personal medical circumstances require extra attention. During your stay, you will go through the following steps:
- You will be given a drug that will soften your cervix. This should take effect for about 60 minutes. While there, you may get a little cold. Therefore, take a warm sweater or cardigan with you.
- You will receive a possible anesthetic of sedation.
- Using a duck’s mouth, Art inserts a small tube into your uterine cavity. Through that tube, your uterus is drained. This suctioning takes about 1 to 2 minutes. If you are more than 13 weeks pregnant, the procedure takes 10 to 20 minutes.
- You will go to the rest room where you should stay for about half an hour. The nurse will stay near you. If everything goes normally, you can then go home.
- After a curettage with sedation, you may not drive your own car or bicycle home. Nor should you travel alone by public transportation. So ask for someone to pick you up. If you have had a complicated anesthetic, you can mostly drive home by yourself or take public transportation.

If you wish, we will place an IUD immediately after the curettage with sedation. You bring the IUD yourself or buy it from us. Sometimes your health insurance company will reimburse the cost of the IUD. Check this before you have the IUD placed with us. Have we already placed the coil in your home and your insurer does not reimburse the cost? Then you cannot reclaim reimbursement from Mildred Clinics.
Placing an IUD is not free either. Only you don’t have to pay this cost yourself if you have it placed after a termination of pregnancy. If you don’t have to pay the cost of your abortion treatment, then you don’t have to pay the cost of the IUD placement either.

Step 4
Taking it easy
We recommend that you take it easy for a few days after the treatment. You probably won’t have much pain, but you may feel a little weak. If you do feel pain, you can take a painkiller, such as Paracetamol or Ibuprofen. You will be given a short course of antibiotics to prevent infection.
In the first few weeks, you usually still have some blood loss and brownish discharge. You also often lose some clots, thick pieces of blood. Sometimes these clots are so big that they worry you. But these clots are quite normal.
Due to risk of infection, you should not use tampons, have sex, swim or bathe for the first week. It is also best not to play sports or engage in strenuous physical activity yet.
After curettage, you can get pregnant again immediately. Therefore we advise you to start using contraception right away. If you are on the pill, it is best to start on the day of the curettage or the day after. After 4 to 6 weeks you will have your first period. Day 1
First tablet in the clinic
Treatment with the abortion pill is possible if you are less than 9 weeks pregnant and the doctor considers it medically justified. This treatment requires you to take several tablets, spread over 3 days. This causes the foetus to be rejected and you will have an early miscarriage.
You will take the first tablet at the clinic immediately after the intake interview. This is a drug that inhibits the growth of your pregnancy. You take the tablet by mouth with a little water. Then you can go home and get the remaining tablets.
Usually you don’t notice anything from this first tablet, but some people get some stomach discomfort. You may also experience abdominal cramps, sometimes with some bleeding. Taking a break
The day after your first tablet, you can work or do other things as usual. You may start to lose some blood at the end of the day. Very sometimes you will lose the amniotic fluid by then. Day 3
Inserting four tablets and staying home
On the third day, you will stay at home all day. It is not necessary for someone to be with you, we recommend it. Someone who can assist you and possibly take care of your children. Also make sure you have transportation available, in case you need to go to the clinic or doctor unexpectedly. If you are a minor, an adult must be present on this day.
On this day, you take the following steps:
- Preferably after breakfast, go to the toilet and empty your bladder. If the morning is not convenient, you may also insert the tablets later in the day, again, first go to the toilet and empty your bladder. Please note that no consultation with our clinic is possible during the evening and night in case of questions.
- Lie on your bed and insert through your vagina the 4 tablets you were given by the clinic.
- Stay in bed for half an hour to an hour to allow the tablets to take effect.
- Afterwards, take one or more painkillers to prevent you from getting a lot of abdominal cramps. Which painkillers you can take is written on the brown envelope you got from the clinic.
- Then you can get up and do whatever you want as long as you stay home. You may not feel very well; in that case, take it easy. Walk around regularly to activate the process.
- After 4 to 6 hours, the amniotic sac is usually expelled. This usually happens when you are on the toilet. This is often accompanied by blood loss. You also often lose clots, thick pieces of blood. In addition, you may have abdominal cramps and nausea. Sometimes the amniotic sac is expelled earlier or later; this is different for each woman. The amniotic sac looks like a small, whitish cotton ball with flakes.
- Over the course of the day, the heavy bleeding and pain will subside. If this does not happen and you continue to bleed very heavily for hours? If so, contact your doctor or the clinic.

After day 3
Blood loss slowly decreases
After the day you took the pills, you continue to lose blood for some time. Sometimes this lasts a week, but can also last until the next period.
You get your first period after 4 to 6 weeks. If you are on the pill, your period starts after the first strip (3 weeks). Even if you do not normally take the pill, we recommend that you do so now. This allows the cycle to return to normal.

If you live or work legally in the Netherlands, the cost of an abortion is fully reimbursed by the Ministry of Health, Welfare and Sport. (See BSN and identification)

Bringing someone
You may bring a companion with you to the intake interview. However, the doctor and/or nurse will always speak to you alone, to make sure you make your choice freely.

Driving
If you have had a curettage with sedation, you should not participate in traffic for 24 hours afterwards. So it is important that someone accompanies you home after the treatment. Even if you walk or take public transportation home, it is wise to do so with accompaniment.

Food and drink
For curettage with local anesthesia and for treatment with the abortion pill, you can eat and drink normally. But if you get a curettage with sedation, you may not eat, eat sweets or smoke from 6 hours before the treatment. You may only drink 1 glass of water. From 2 hours before the treatment this is also no longer allowed. Because of this, you will probably feel a little weak after the treatment. Take some food and drink with you for after the treatment. Under the age of 16?
If you are 12 years old or younger, you can get an abortion only with the consent of your parents or guardians. Are you between the ages of 12 and 16? Then both your parents or guardians and you yourself must give permission.
Do your parents not want to give permission or are you unable to discuss this issue with them? Then contact us anyway. In exceptional cases, the doctor may still decide to carry out the treatment. Complications
It is normal to continue losing blood for some time after curettage from the abortion pill. This often includes clots, thick pieces of blood. There is no need to worry about that. But is the bleeding very heavy (4 pads per hour for at least 3 hours in a row)? Or do you have a fever of more than 38°? Then we advise you to call us or your doctor.
Our nurses will give you a discharge letter after treatment and a form with all kinds of additional information during the period after the hold.
